Have You Heard of Fortify?

Pediatric Associates of Charlottesville (PAC) is an independent, doctored-owned, private practice established in 1971.   Our practice belongs to Fortify Children’s Health, a non-profit child-focused network of pediatric providers dedicated to delivering quality care to children across Virginia. Fortify uses patient data to measure quality metrics, such as immunizations rates, and identify patients who may be at risk of complications from chronic medical conditions.  Fortify ensures that its network pediatricians strive to meet the quality metrics and provide care coordination for patients with chronic conditions.

 

What is care coordination?  This is care delivered by professional personnel, primarily registered nurses, trained specifically to help families receive the care they need for a variety of medical conditions.  Fortify care coordinators come in two flavors: those hired directly through Fortify and those hired by PAC.  The care coordinators reach out to qualifying families proactively and regularly to make sure they have all their needs met, including the correct medications, care plans, social supports, and follow up for the chronic conditions.

 

How does this affect you?  As a patient of PAC, you will likely not notice any difference in your care.  Fortify works in the background with your pediatrician to make sure you receive quality care.  If you have a chronic medical condition and you qualify for care coordination services, then you may receive a call from our care coordinators on a regular basis. Fortify and PAC have worked together to design care coordination to make sure your child is getting the highest quality care possible.  This costs you nothing, nor do you need to sign up for this – it is part of your regular care at PAC.  Fortify will not try to sell you any product or service.  Fortify does not share your data with any other entity, and your data remains protected by federal and state privacy laws.  If you ever have questions or comments about care coordination, please speak to your pediatrician or visit www.fortifychildrens.org.
